About CONNECT Partners
At CONNECT Partners, we support people with complex disabilities and health needs to live in their own homes at Cambie Gardens and build lives of meaning, connection and choice.
Our Life Redesign Model is grounded in people's strengths, hopes and aspirations, and in the belief that life is about having people to love, things to do, places to belong and opportunities to contribute.
About the Role
We're looking for a Service Access & Transitions Leader who enjoys bringing people together, building relationships and finding possibilities.
You'll work closely with Vancouver Coastal Health and other partners to support thoughtful, well-coordinated transitions into and, when appropriate, from Cambie Gardens. You'll help people and those important to them navigate change, build connections and settle into a new chapter of their lives.
But moving in is only the beginning. A significant part of your role will be helping bring CONNECT’s Life Redesign Model to life, every day. You'll work alongside the people we support and our coaching teams to understand what matters to each person, what they hope for and what might be possible next.
You'll lead the ongoing development of Life Redesign practice, helping teams turn hopes and aspirations into meaningful everyday experiences, supporting a continuing process of discovery, action, reflection and change.
You'll also help people build connections beyond their front doors - to find places where they belong, develop friendships, pursue interests and contribute to their communities. Being a good neighbour goes both ways, and you'll help create opportunities for those relationships to grow.
What You'll DoBuild relationships and support meaningful transitions
· Lead service access and referrals in partnership with Vancouver Coastal Health, supporting thoughtful, timely and well-coordinated transitions.
· Build trusted relationships with the people we support, their families and others important to them.
· Bring people and partners together to navigate challenges and support transitions into, throughout and from Cambie Gardens.
Help people redesign their lives
· Champion CONNECT's Life Redesign Model, keeping people's hopes, choices and priorities at the heart of what we do.
· Lead and strengthen an ongoing, person-led approach to Life Redesign conversations, planning, action and review.
· Work alongside coaches to build shared practices that turn possibilities into meaningful participation in everyday life.
· Encourage curiosity, reflection and learning, helping teams recognize what's working, what might need to change and what could be possible next.
Build community and strengthen partnerships
· Help people develop and sustain meaningful connections in their neighbourhoods and wider communities.
· Look for opportunities for people to participate, contribute and be recognized as neighbours and community members.
· Build collaborative relationships with families, advocates, funders and community partners.
Lead, learn and help us grow
· Work collaboratively with the Leader of Culture, People & Services and Wellness Integration Coach.
· Contribute to the ongoing evolution of Life Redesign practice across CONNECT, bringing ideas, learning and experience from CONNECT Partners into our shared efforts to keep getting better.
· Help us understand and share the difference we're making in people's lives, using people's experiences and meaningful outcomes to guide learning and improvement.
What You Bring
You're someone who sees people first, builds relationships naturally and believes meaningful change happens when people work together.
You bring experience in community, disability, rehabilitation, health or human services, along with strengths in relationship-building, problem-solving and helping others grow.
You're comfortable navigating partnerships, bringing people together around shared goals and following things through. You bring curiosity, sound judgment and a willingness to challenge assumptions and explore new approaches.
Above all, you believe people have something to offer, everyone deserves to belong, and there's always more to discover about what's possible.
Working Together
This is a hands-on, relationship-centred leadership role based at Cambie Gardens, with flexibility to respond to the needs of the people we support, our teams and our partners.
You'll report to the Director, Leading Practice & Life Redesign and work alongside the Leader of Culture, People & Services and Wellness Integration Coach. Each leader brings distinct responsibilities, while working together to support people, strengthen our coaching teams and ensure our services are responsive, safe and continually improving.
At CONNECT, we value kindness, collaboration, accountability and learning.
If you're energized by building relationships, finding possibilities and helping people create lives of meaning and connection, we'd love to hear from you.
